MMP Card (Medical Marijuana Program)
Qualifications
- Must be a Connecticut Resident
- Must be treated for a debilitating medical condition by a connecticut- licensed physician, advanced practice registered nurse, or physician assistant

- Cannot be an inmate in a correctional institution or facility under the supervision of the Connecticut Department of Corrections
- Must be 18 years or older
- Must bring in or upload the current (within one year) medical records pertaining to the qualifying medical conditions to info@thecannabisconsult.com
Cost
- Certification: The initial visit is $210.00 and for Veterans $175.00
- Recertification: The visit is $155.00 and for Veterans/ Disability is $125.00.
- Recertification should start in the eleventh month prior to expiration if you want to recertify.
- MMP State Registration: $100.00
Process
- Make an appointment with a certified medical marijuana prescriber either in person or virtual
- Fill out the certification/ recertification forms and submit in person or upload information at info@thecannabisconsult.com
- Bring or upload the current medical records pertaining to the qualifying medical condition.
- Pay invoice on Square Up
- Complete the certification/ recertification process
- If a primary caregiver is needed:
- A qualifying patient can assign primary caregiver to assist with purchasing products for qualifying patients
