MMP Card (Medical Marijuana Program)

Qualifications

• Must be a Connecticut Resident
• Must be treated for a debilitating medical condition by a connecticut- licensed physician, advanced practice registered nurse, or physician assistant

• Cannot be an inmate in a correctional institution or facility under the supervision of the Connecticut Department of Corrections
• Must be 18 years or older
• Must bring in or upload the current (within one year) medical records pertaining to the qualifying medical conditions to info@thecannabisconsult.com

Cost

1. Certification: The initial visit is $210.00 and for Veterans $175.00
2. Recertification: The visit is $155.00 and for Veterans/ Disability is $125.00.
   1. Recertification should start in the eleventh month prior to expiration if you want to recertify.
3. MMP State Registration: $100.00

Process

1. Make an appointment with a certified medical marijuana prescriber either in person or virtual
2. Fill out the certification/ recertification forms and submit in person or upload information at info@thecannabisconsult.com
3. Bring or upload the current medical records pertaining to the qualifying medical condition. 
4. Pay invoice on Square Up
5. Complete the certification/ recertification process

• If a primary caregiver is needed:
  • A qualifying patient can assign primary caregiver to assist with purchasing products for qualifying patients
